Documents Required for Benefit and Entitlement Claims

A life-changing event such as marriage, divorce, birth or adoption of a child or a death, affect the military service member and family members in many ways. The following lists of documents are used in assessing final determination of benefits and entitlements for Air Reserve Component (ARC) members and their families. As a military member you are required to be ready for deployment, keep personal information updated, provide adequate support for dependents and provide survivor benefit information to family members in the event of your death.

Note: Ensure surviving spouse, children or family members are aware of document location(s).

Required Documents

Having the following documents available will greatly enhance the services provided to eligible family members or beneficiaries:

  • Birth, Marriage, and Death Certificates
  • Divorce Decree
  • Last Will and Testament
  • Adoption Papers
  • DD Form 214 and/or NGB Form 22
  • Retirement Order
  • Notification of Eligibility for Retired Pay (NERP)/20-Year Letter
  • DD Form 2656-5, Reserve Component Survivor Benefit Program (RCSBP) election form
  • Insurance Policies
  • Casualty notification telephone number (720-847-3611/3610)
  • Family Care Certification form, AF IMT 357
  • Emergency Data Form - vRED (obtain copy from vMPF)
  • SGLI Form (SGLV Form 8286)

Marriage

Documents required for Military Personnel Record Update, DEERS Enrollment, and Identification Card include:

  • Marriage License or Certificate
  • Finalized Divorce Decree (if applicable)
  • Last Will and Testament or Prenuptial Agreement
  • Birth Certificates (to include those of children acquired through the marriage)
  • School certification for full-time student between ages 18-22 or up to age 23
  • Medical statement for child disabled prior to age 18

Note: Must be certified originals, court-certified copies, or certified photocopies

Birth or Adoption of a Child(ren)

Documents required for Military Personnel Record Update, DEERS Enrollment, and Identification Card include:

  • Birth Certificate
  • Last Will and Testament
  • Adoption or Custody Documents

Death of Member (As applicable)

Documents required, as applicable, for the death of a members in either a participating or nonparticipating status include:

  • Final Death Certificate
  • DD Form 1300, Report of Casualty (depending upon duty status)
  • Last Will and Testament
  • Emergency Data Form - vRED or DD Form 93, Record of Emergency Data
  • Birth Certificate of member, when applicable
  • Marriage License or Certificate (If marriage information on death certificate is contradictory)
  • Final Divorce Decree (Including Property Settlement)
  • Birth Certificate for child/children
  • School certification for full-time student ages 18-22 (up to age 23)
  • Medical statement for child disabled prior to age 18
  • Certified court conservatorship or guardianship documentation
  • Certified court appointment of executor of estate
  • Affidavit of residency (if surviving spouse is a foreign national)
  • Insurance policies
  • Police report, court verdict or coroner's report
  • Prior year's tax forms

Death of Retiree (As applicable)

Documents required, as applicable, for the death of a retired member awaiting pay or receiving Retired Pay include:

  • Final Death Certificate or DD Form 1300, Report of Casualty
  • Last Will and Testament
  • Birth Certificate
  • DD Form 214/215 and/or NGB Form 22/22a
  • Retirement Order
  • Marriage License or Certificate (If marriage information on death certificate is contradictory)
  • Final Divorce Decree (Including Property Settlement)
  • Birth Certificate for child/children
  • School certification for full-time student ages 18-22 (up to age 23)
  • Medical statement for child disabled prior to age 18
  • Certified court conservatorship or guardianship documentation
  • Certified court appointment of executor of estate
  • Affidavit of residency (if surviving spouse is a foreign national)
  • Insurance policies
  • Servicemember's Group Life Insurance (SGLI) Certificate (SGLV 8286)
  • Spouse drawing SBP from a previous member must provide a letter indicating from which member elects continued SBP payment (typically the highest paid)
  • Police report, court verdict or coroner's report
  • Prior year's tax forms

Death of Dependent

Documents required for Military Personnel Record Update, DEERS Enrollment, Identification Card, and Family Servicemembers’ Life Insurance (FSGLI) Claim include:

  • Certified copy of the death certificate
  • It may become necessary to submit other evidence, upon notification
