General and Mrs Jerome F. O'Malley Award

This award is named in honor of the late General and Mrs. Jerome F. O'Malley, former commander, Tactical Air Command. It recognizes the wing commander and spouse team whose contributions to the nation, the Air Force, and the local community best exemplify the highest ideals and positive leadership of a military couple serving in a key Air Force position. The military member must have served as a wing commander at least six months during the award period and must have at least two years retainability to be eligible for consideration.

Nomination Authority

MAJCOM/CC, MAJCOM/CCC, MAJCOM/A1, COCOM/J1, FOA, and DRU

The period of service for the award is 1 January - 31 December.

Organizations and base-level personnel must contact their Major Command (MAJCOM), Combatant Command (COCOM), Field Operating Agency (FOA), Direct Reporting Unit (DRU) or MAJCOM equivalent for applicable suspense date and for additional information regarding nomination procedures.

Nominations must include specific acts and examples that identify the couple as leaders and spokespersons for the Air Force. Ensure the write-up focuses on what the nominees did as a team and the results of their accomplishments as a team. Be sure the write-up focuses equally on the contributions made by the spouse. Include specific examples in the following areas:

  • Leadership: Cite innovation and ingenuity in problem-solving and in developing systems, programs, and ideas that positively impacted the mission, morale, and welfare of Air Force or wing activities and personnel. The leadership abilities of both the wing commander and spouse must clearly demonstrate professionalism, integrity, morality, and excellence in inspiring others by personal example, conduct and character
  • Positive and articulate spokespersons for the Air Force: Show how the couple projected leadership into the local community as evidenced by the wing commander's and spouse's assumption of leadership roles and participation in social, cultural, religious, and other wing or community service activities
  • Other accomplishments: List other achievements or accomplishments which set the wing commander and the spouse apart from others of similar responsibilities. Include unique contributions to the nation, Air Force, wing, base, or community

Nomination Package

Write-up

A write-up, limited to 30 lines (including headers, which must stand alone from the bullets), single-spaced, size 12- point font, using only the AF Form 1206, nomination for award (MUST USE THE CURRENT AF FORM 1206 VERSION THAT IS ON E-PUBS). Bullet format is mandatory. Use the headers cited in the above paragraphs 4a-4c.

Only common acronyms known throughout the Air Force will be authorized, (i.e.; AF, USAFE, ACC, CC, TDY, NCO).

Public Release Statement

A Public release statement addressed to Air Force Personnel Center Recognition Programs Section (AFPC/DP1SSP): Each nominee must sign a public release statement that reads verbatim "I do or do not (circle one) agree to the use of Privacy Act information in the nomination narrative. This information may include Privacy Act information or Personally Identifiable Information (PII) found in DoD 5400.11-R, DoD Privacy Program, Disclosure of Personal Information to Other Agencies and Third Parties and guidance in Air Force Instruction 33-332, Air Force Privacy and Civil Liberties Program, Chapter 1, 2, and 6. I understand those transmitting person information via email will exercise caution and adequately safeguard it in accordance with AFI 33-332, paragraph 2.5. and subparagraphs. The announcement message or any publicity regarding the award nomination will contain no privacy act information other than name, rank, and base of assignment." The nominee must sign and date this statement.

Endorsement Letter

An endorsement letter signed by the MAJCOM/COCOM/FOA/DRU commander, vice commander, or executive director stating that validation with the nominee's unit was accomplished ensuring that the nominee does not have any derogatory information, also stating verbatim that: “A review of the official personnel record was conducted on (date) and revealed no information that would bring discredit to the award or the United States Air Force during the award period.” This may be delegated to the A1 Director by the MAJCOM/COCOM/FOA/DRU commander.

Biography

A “joint” biography, limited to one single-spaced typewritten page (see AFI 36-2805, Attachment 2, for format).

Citation

A citation limited to no less than 50 and no more than 70 words (not including opening and closing statements). The citation should include specific examples of the act or services provided. Refer to AFI 36-2805, Special Trophies and Awards, attachment 3, for format.
Acronyms are not authorized in the citation.

Nomination Submission

Each MAJCOM/COCOM/FOA/DRU may submit only one nomination. Please send nominations in original format to AFPC/DP1SSP (AFPC.DP1SP.RecognitionProcess@us.af.mil). The only scanned documents we will accept are those documents with signatures, such as endorsement letters and public release statements. The AF Form 1206 must be sent in its original format (NOT SCANNED) and all other documents should be in word document format.

If nomination package(s) is not received by the established suspense date and an extension has not been approved, the nomination package will not be considered and will be returned with no action.
