Military Personnel Record or Official Document Requests

The Master Personnel Record Group (MPerRGp) contains all permanent military personnel documents such as the DD Form 214, Certificate of Release or Discharge from Active Duty, DD Form 4, Enlistment or Reenlistment Agreement - Armed Forces of the United States Oaths of Office, special orders, performance reports, etc and is electronically maintained in the Automated Records Management System (ARMS).

HQ Air Reserve Personnel Center (ARPC) maintains the master personnel record for all Air National Guard (ANG) and Air Force Reserve (AFR) Airmen regardless of status. The Officer Selection Record (OSR), which is part of the MPerRGp, is created and maintained by HQ ARPC (Promotion Secretariat Division) for all ANG and AF Reserve officers.

The military service records for Veterans, Next of Kin (NOK) of a deceased veteran, and Retirees can be obtained through a couple of kind agencies outlined below.

Military Record and/or Official Document Requests - Unit Assigned

Copies of military personnel records or documents can be obtained by various processes.

  • Unit Participating ANG or AF Reserve - Unit members have the capability to access and retrieve some military personnel documents by accessing ARMS or Personnel Records Display Application (PRDA) (requires Common Access Card (CAC) and reader) through the Air Force Personnel Center Secure (AFPC) Applications
  • Unit participating ANG or AF Reserve without CAC access. Unit members without CAC access have the capability to request military personnel documents

Military Record and/or Official Document Requests - Retired, IRR, Veterans, NOK

Copies of military personnel records or documents can be obtained by various offices depending on your past military affiliation.

  • Retired Reserve (not receiving pay) and Individual Ready Reserve (IRR) - IRR members may submit a request for service record documents through this website using the 'E-mail Us' link located below. The Standard Form (SF) 180, Request Pertaining to Military Records is required for a DD Form 214/215 or a complete military personnel record
  • Discharged or Retired (receiving pay) - Military personnel records for those discharged or retired (receiving pay) BEFORE 1 October 2004, may be located at:

  • Veterans or Next-of-Kin (NOK) of a deceased veteran - Requests from veterans or deceased veteran’s NOK may be submitted online by using eVetRecs
  • Retired Reserve (not receiving pay) and Individual Ready Reserve (IRR) - IRR members may submit a request for service record documents through this website using the 'E-mail Us' link located below. The Standard Form (SF) 180, Request Pertaining to Military Records is required for a DD Form 214/215 or a complete military personnel record

Requests to NPRC for military service records must be submitted on the SF 180. Please note, on the SF 180, under Section II, item 1, do not check 'All Documents in Official Military Personnel File (OMPF)' and 'Medical Records', if you only want a copy of a specific document (i.e. DD Form 214). List specific document(s) individually under the 'Other (Specify)' box.

National Personnel Record Center (NPRC), St. Louis, MO

The NPRC can be contacted at 314-801-0800 or fax requests to 314-801-9195. The preferred method for requesting documents from NPRC is to fax your request using the Standard Form 180 with the following information: name, social security number and service number (if applicable), date of birth, current e-mail and mailing address, day time phone number, service dates, place of service, year of requested document, and requested documents.

NOTES:

1. Please clarify the specific military document requested (i.e. DD 214).

2. If retiring or separating in the near future, make sure you print or download a copy of your military personnel records prior to your effective date, thereafter you will no longer have access to ARMS or PRDA.

Air National Guard State/Territory Military Records

A copy of your state/territory military personnel records are maintained by your state/territory Joint Force Headquarters (JFHQ). You can contact them directly to obtain documents from your state/territory military records.

Requesting a Document from the myPers - Total Force Service Center (myPers - TFSC)

Submit a 'Support Request' in order to obtain a copy of the following documents ('Email Us' (CAC required) to do so):

  • DD Form 214 (SF 180 required)
  • DD Form 215, Correction to DD Form 214, Certificate of Release or Discharge from Active Duty (SF 180 required)
  • Enlisted Performance Report (EPR)
  • Officer Performance Report (OPR)
  • Awards and Decorations
  • Certificates, orders and/or citations
  • Current Awards and Decorations - Listing Only
  • Current Retirement Points
  • Mortgage letter
  • Reserve Component Survivor Benefit Plan (RCSBP) election
  • 20 year letter copy

Submitting a Support Request - myPers

To submit a new ‘Support Request’ to the myPers - TFSC for assistance use E-mail Us (CAC required). Simply follow these steps:

  1. E-mail Us (CAC required)
  2. Ensure ‘Component’ and ‘Status’ are correct.
  3. Under ‘Category,' select the applicable personnel program subject for your request.
  4. In the ‘Subject’ box, enter the title of your request, and then the specific assistance you need in the ‘Question’ box.
  5. Upload and attach any documents to support your request in the ‘Attach Documents’ box.
  6. Click the ‘Continue’ button.
  7. Review any ‘Preliminary Responses’ provided.
  8. Finally, click the ‘Finish Submitting’ button to forward your support request to the myPers - TFSC for assistance.

To Check the Status of your Request Submitted through myPers

  2. Under the 'My Account' section on the left, click the 'Incidents/Messages' or 'My Documents' links to check the status or your request and/or view and print/save your requested documents.

Removing Erroneous Documents from your Military Records

To remove erroneous documents from your military personnel record (ARMS/PRDA), please contact HQ ARPC by using 'Email Us' or calling the myPers-TFSC and include information, type of problem, form, document and/or item number.
