Suspense to submit complete packages to Holm Center: 30 November
The purpose of this message is to solicit nominations for the Air Force Reserve Officer Training Corps (AFROTC)/Officer Training School (OTS) Distinguished Alumni Award.
Nominations will be accepted directly from the nominee his/herself or from a second-party nominator.
This award honors those that have accomplished great achievements after graduating from AFROTC, OTS (BOT, COT, RCOT or AMS), School of Military Sciences-Officer (SMSO), Officer Commissioning School (OCS), Aviation Cadets, Military Indoctrination for Medical Service Officers (MIMSO), Health Professionals Officer Indoctrination Course (HPOIC), or Air Force Officer Orientation Course (AFOOC). Applicant Qualifications
Applicant must have graduated from AFROTC, OTS (BOT, COT, RCOT or AMS), School of Military Sciences-Officer (SMSO), Officer Commissioning School (OCS), Aviation Cadets, Military Indoctrination for Medical Service Officers (MIMSO), Health Professionals Officer Indoctrination Course (HPOIC), or Air Force Officer Orientation Course (AFOOC). NOTE: Applicants must be retired or separated from the military. Current Active Duty, Reserve, and Guard members are ineligible for this award.
Applicants must have achieved one of the following:
- Attained the rank of Lieutenant General or above for line officers. Attained the rank of Major General or above for non-line officers
- Awarded the Silver Star or higher precedence medal
- Attained the rank of Lieutenant General or above for line officers. Attained the rank of Major General or above for non-line officers
- Attained a high level of honorable recognition or distinction for an accomplishment with Air Force-wide or national/international significance (e.g. certain authors, astronauts, test pilots, etc.) or for historically notable service or combat achievements (e.g. "Ace" status, most air-to-air victories during a campaign, first officer of x type to reach a certain rank, etc.)
If civilian, applicant must have achieved all of the following:
- Completed a minimum of four years of active duty, Air Force Reserve, or Air National Guard, or a combination thereof that equals four or more years of service
- Attained a high level of honorable recognition (Pulitzer Prize winner, public figure such as a U.S. Congressman, Governor, etc.) or distinction for an accomplishment of national/international significance (significant medical discovery, etc)
- Maintained an honorable example in military and civilian life
Nomination Packages
All nomination packages must include:
- Official AF documents that prove honorable service or discharge (recommend DD Form 214,Certificate of Release or Discharge from Active Duty or equivalent)
- As many documents as possible to build background of accomplishments on applicant for the AFROTC Commander and the OTS Commandant to consider. Examples include distinguished awards won, highest decorations achieved, combat tours, etc.
- An official photograph (color, if possible) in highest rank held (if military)
- AF Form 1206, Nomination for Award with a maximum of 25 lines describing accomplishments and contributions of applicant
- A suggested citation for the plaque
Additional Information
- Nominations and selections may be posthumous
- NLT 30 November 2017, submit all documentation with a cover letter requesting induction to Holm Center/CCX
- OTS/CMDT and AFROTC/CC will review respective nominations for accuracy/distinction
- After all nominations have been received and reviewed, AFROTC and OTS will convene selection boards to recommend inductees to Holm Center/CC (final authority on all induction decisions)
